To the Alumni of the FIK-UMS Public Health Study Program, Praise Thank God, on 9,10 and 11 November 2017, the Regional Government of IAKMI Central Java and several Regional Governments of Health Professional Organizations (OP) in Central Java, coordinated by MTKP Central Java together with MTKI, have held On Site with One Day Service (ODS) issuance of STR for Health workers in the Central Java region.

In the implementation of the ODS, the IAKMI Central Java Regional Government prioritized and proposed the issuance of STRs for proposers whose STRs had not been issued in the previous period. Just a few new suggestions both online and manually.
So, for SKMs who have proposed an STR and have not yet published it, (with a note via Pemcab and Pengda) it can be seen in this list of ODS STR lists.

A.Collectively through Dismemberment
1. Pengcab registers and makes a List of STR to be taken. Download list format here.
2.Copy of ID card
3. Copy of Temporary KTA/KTA.
4.Send / email first before collection.

B.Individual
1.Fill out the download form here

3.Copy KTA / KTA Temporary IAKM

4 If it is taken by someone else, it must be with a power of attorney.
